Insurance Patients – Life Align is a preferred provider for most insurances. Please bring your insurance information and a picture ID to our office so that we can call and verify your coverage.

Medicare Patients - Dr. Cherniawski is a provider for Medicare.  Medicare does not cover the cost of the initial examination but does cover a portion of treatment.

Cash Patients – Exam prices start at $45 and treatment starts at $30.

Consultations – Life Align provides a $20 consultation and screening to those who are interested to find out whether or not our services can help without the commitment of completing a full exam.
